1. The Subject Of The Order Is An Investment Task Entitled: Reconstruction And Thermal Modernization Of The Sgzoz Building In Jaworze Along With The Construction Of An External Elevator. The Aim Of The Investment Project Is A Comprehensive Renovation And Modernization Of The Building Of The Independent Municipal Health Care Center In Jaworze, Located At Ul. Lecznicza 272, On Plot No. 1172/27. The Building Is A Public Service Facility - It Serves As A Health Clinic (Facility Category: Xi - Health Service Buildings, Clinics). The Planned Activities Are Aimed At Improving The Facilitys Energy Efficiency, Functionality, Safety And Aesthetics. The Investment Will Contribute To Improving The Quality Of Medical Services Provided And The Comfort Of Patients And Staff.2. The Scope Of Works Includes Construction, Finishing, Installation And Thermal Modernization Works, Including: 1) Replacement Of External And Internal Windows And Doors With New Elements With Improved Thermal And Acoustic Insulation Parameters - With A Heat Transfer Coefficient Uo = 1.1W/M2k. And Acoustic Insulation At A Minimum Level. 40Db;2) Replacement Of Floors With Pvc Floor Coverings In Utility Rooms And Ceramic Tiles In Passageways;3) Installation Of New Cement Screeds, Gypsum Plasters And Painting Walls With Wash-Resistant Paints;4) Insulation Of External And Foundation Walls Along With The Implementation Of Perimeter Drainage To Improve Moisture Protection;5) Replacement Of The Existing Roofing Made Of Felt Felt With A High-Durability Roofing Membrane Using A Layer Thermal Insulation Made Of Eps 100 Polystyrene;6) Renovation Of The Central Heating And Electrical Installations, Adapting Them To Current Safety And Energy-Saving Standards;7) Adapting The Facility To Applicable Regulations Regarding Accessibility For People With Disabilities By Building An External Passenger Lift (Elevator) In A Reinforced Concrete Structure, Together With Thermal Modernization Of The Building And Construction Of A New Façade Made Of White Or Light Gray Plaster.3. Intended Use Of The Building: After Completing The Renovation Works, The Building Will Continue To Function As A Health Clinic - A Public Health Care Facility Providing Medical Services In The Field Of Primary Health Care And Selected Specialist Services. The Intended Use Includes: 1) Serving Patients In Medical And Treatment Rooms; 2) Conducting Medical And Administrative Activities By Staff; 3) Ensuring Safe, Hygienic And Comfortable Conditions Of Stay In The Facility; 4) Maintaining The Continuity Of The Facilitys Operation In Accordance With Applicable Sanitary, Fire Safety And Construction Regulations. Utility Program Provides For The Operation Of: 1) Medical And Treatment Rooms, 2) Nursing Stations, 3) Patient Registration And Waiting Rooms, 4) Hygiene And Sanitary Rooms, 5) Technical And Storage Rooms, 6) Administrative And Office Rooms.
