The Subject Of The Order Is The Purchase Of Medical Equipment (Infusion Pumps With Accessories) Along With The Maintenance Of Devices After The Warranty Period For Copernicus Pl Sp. Z O. O. The Subject Of The Order Consists Of 1 Part, Which Is Divided Into 6 Tasks: 1) Task No. 1 - Set Of Infusion Pumps With Docking Stations, 2) Task No. 2 - Set Of Infusion Pumps With Docking Stations, 3) Task No. 3 - Infusion Management System, 4) Task No. 4 - Maintenance Of Devices From Tasks 1, 2, 3, 5) Task No. 5 - Syringe Infusion Pump, 6) Task No. 6 - Successive Deliveries Of Accessories For Pumps From The Above-Mentioned Tasks. 2. The Detailed Scope Of The Subject Of The Order, List Of Equipment, Quantities And Method Of Execution Of The Order Are Included In The Annexes To The Swz, Namely: 1) Attachment. No. 1A To Swz - Price Form, 2) Attachment. No. 1B To Swz - Description Of The Subject Of The Order, 3) Attachment. No. 3A To Swz - Draft Contract For Successive Delivery (Applies To Tasks 1-3, 5), 4) Attachment. No. 3B To Swz - Draft Contract For Successive Delivery (Concerns Task 6), 5) Attachment. No. 3C To Swz - Draft Contract For Equipment Maintenance (Concerns Task 4). The Contractor Attaches Appendix No. 1A And 1B To The Swz To The Offer. By Completing The Indicated Items In Annex No. 1B To The Swz-Opz, The Contractor Confirms That It Meets The Requirements Specified By The Ordering Party. 3. The Offered Items Must Be Complete, Of A High Standard Both In Terms Of Quality And Functionality, And Free From Material, Construction And Legal Defects. 4. The Offered Subject Of The Order Must Be Approved For Trading And Use In Accordance With Applicable Law. 5. The Subject Of The Order Must Be Consistent With The Act Of April 7, 2022 On Medical Devices (Consolidated Text: Journal Of Laws 2022.974 Of 2022/05/09). 6. Regarding Task No. 4: 1) Pursuant To Art. 95 Section 1 Of The Public Procurement Act, The Ordering Party Requires The Contractor Or Subcontractor To Employ Under An Employment Contract Within The Meaning Of The Act Of June 26, 1974, Labor Code, Labor Code (Journal Of Laws Of 2019, Items 1040, 1043 And 1495) Continuously Throughout The Duration Of The Agreement, Persons/Persons Performing Service Activities Related To The Implementation Of The Subject Of The Order. 2) No Later Than The Conclusion Of The Contract, The Contractor Will Provide The Ordering Party With A Declaration Of Compliance With The Requirement Referred To Above, I.E. That The Person/Persons Performing Service Activities Related To The Implementation Of The Subject Of The Order Will Be Employed Under An Employment Contract Within The Meaning Of The Act Of June 26, 1974, Labor Code. 3) At Each Stage Of Contract Implementation, The Ordering Party Has The Right To Verify The Contractors/Subcontractors Declaration (Concerning The Form Of Employment Of The Persons Referred To Above). For This Purpose, He Is Entitled In Particular To Request The Competent Labor Inspectorate To Conduct An Inspection. 7.
