Grottgera 4 In Lublin, Ul. Czechowska 19 In Lublin, Ul. Karłowicza 4 In Lublin, Ul. Stefczyka 3B In Lublin, Ul. M. Skłodowskiej-Curie 3 In Lublin, Ul. Uniwersytecka 4 In Lublin, Ul. Wojciechowska 9A In Lublin, Ul. Warszawska 14 In Biała Podlaska, Ul. Mickiewicza 37 In Chełm, Ul. Partyzańskiego 94 In Zamość. 2. Detailed Conditions For The Provision Of Services In Individual Locations Are Specified In The Description Of The Subject Of The Order, Constituting Annex No. 1 To The Swz (Hereinafter Referred To As Opz). 3. Response Time From Sending The Notification To Taking Diagnostic Actions By The Contractor (Remotely Or Locally) 1) Critical Failure: 1 Working Hour; 2) Significant Failure: 2 Business Hours;3) Fault: 4 Business Hours;• Business Hours - On Business Days From 7:00 A.M. To 4:00 P.M.• Business Days - Days From Monday To Friday, Excluding Public Holidays Specified In The Provisions Of Law Generally Applicable In The Territory Of The Republic Of Poland.4. Time To Remove The Failure 1) Critical Failure: Up To 6/8/10* (*According To The Contractors Declaration - It Is A Non-Price Criterion For The Evaluation Of Offers) Clock Hours From The Time Of Reporting. If The Above Deadline Falls Outside The Ordering Partys Working Hours, It Is Postponed Until 8:00 A.M. As Part Of The Order, The Contractor Will Deliver The Following Equipment To The Ordering Partys Headquarters From The Date Of Signing The Contract, And No Later Than April 24, 2026 (I.E. 5 Business Days Before The Date Of Commencement Of The Provision Of Services): 1) 80 Brand New System Phones. The Devices Must Come From The Manufacturer Of The Offered Switchboard Or Be Consistent With The Compatibility List Of This Switchboard (If Applicable - The Contractor Will Provide A Compatibility List Before Signing The Contract) With Parameters Meeting The Minimum Technical Requirements Specified In The Content Of The Opz.2) 5 System Phones Compliant With The Description For The Above-Mentioned Equipment, But Equipped With A Minimum Of 40 Programmable Quick-Dial Buttons Along With Signaling Indicating The Line Status (Blf) - The Use Of An Adapter Extending The Functionality Is Allowed Camera.7.
