1. The Subject Of The Order Is The Provision Of The Service Of A Personal Assistant For A Person With A Disability In The Komorniki Commune In 2026 In Connection With The Implementation Of The Personal Assistant For A Person With A Disability Program - Edition 2026.2, From The Solidarity Fund. The Service Will Be Provided For A Maximum Of 7,640 Hours, Including: 5,200 Hours. For Adults With Severe Disabilities, 2,440 Hours For People With Moderate Degrees Of Disability And Children. 3. The Planned Number Of Hours Of Assistant Services Is 7,640 Hours.4. Planned Number Of Assistants For People With Disabilities – Min. 6 People.5. A Detailed Description Of The Conditions For Implementing The Subject Of The Order Is Included In The Draft Provisions Of The Contract Constituting Annex No. 8 To Swz.6. The Contractor Is Obliged, Throughout The Period For Which The Agreement Is Concluded, To Have Civil Liability Insurance For The Activities Related To The Subject Of The Order For A Guarantee Sum Of Not Less Than Pln 100,000.00 (In Words: One Hundred Thousand Zlotys 00/100), Valid No Later Than From The Date Of Signing The Contract Until The Final Acceptance. The Ordering Party Will Accept The Liability Insurance Document Held By The Contractor In The Scope Of Activities Related To The Subject Of The Order For The Insured Sum Required By The Ordering Party, Which Will Be Concluded For An Annual Period And Will Be Renewable While Maintaining Continuity Of Insurance Coverage Until Final Acceptance. In Such A Situation, The Contractor Is Obliged To Provide The Ordering Party With A Valid Third Party Liability Insurance Document For The Activities Related To The Subject Of The Order No Later Than Within 7 Days From The Date Of Issuance Of This Document.7. Before Commencing The Implementation Of The Contract, The Contractor Will Be Obliged To Provide The Ordering Party With A Certificate Of No Criminal Record From The National Criminal Register Or The Criminal Register Of Another Country And Information About Not Being Included In The Register Of Sexual Offense Perpetrators In The Form Of A Printout Of Information Downloaded From The Register For At Least Six Personal Assistants Of A Person With A Disability, As Well As, At The Request Of The Ordering Party, Documenting The Experience Of The Persons Indicated In Annex No. 9 Of The Swz. 8. Due To The Specificity Of The Subject Of The Order, The Ordering Party Reserves The Right To Change The Number Of People Using Care Services During The Term Of The Contract.9. The Ordering Party Requires The Contractor Or Subcontractor To Employ, On The Basis Of An Employment Relationship, Persons Performing The Following Activities In The Execution Of The Order, If The Performance Of These Activities Involves Performing Work In The Manner Specified In Art. 22 § 1 Of The Act Of 26 June 1974 - Labor Code.
