The Subject Of The Tender Is The Selection Of A Contractor Who Will Perform Construction Works For The Ordering Party, Consisting Of The Renovation Of The General Surgery Department With The Urology Sub-Department, Including The Replacement Of Doors And Installation Of Panels Over Medical Gas Beds, As Well As Minor Construction Works Resulting From The Planned Functions Of The Rooms Of The District Hospital. Dr. Tytus Chałubiński In Zakopane At Ul. Kamieniec 10, 34-500 Zakopane.2. The Scope Of Construction Works That Are The Subject Of This Contract Includes: 1) Demolition Works, 2) Construction Works, 3) Electrical Installation Works Consisting In The Installation Of 15 Sockets On The Skirting Board Along With A Fuse Panel Above The Door In The Room For Parking Medical Devices Requiring Continuous Power Supply And Completing The Installation According To The Bill Of Quantities, 4) Performance Of Works In The Field Of Securing Plastic Floors And Handrails And Bumper Strips, 5) Performance Of Works In The Scope Of Painting,6) Replacing The Suspended Ceiling,7) Carrying Out Finishing Works,8) Completing The Medical Gas Installation By Delivering And Installing Medical Gas Panels For 36 Beds (Double And Single, As In The Attached Views), In Accordance With The Technical Documentation Constituting Annex No. 7 To The Swz, In Accordance With The Technical Specification For The Execution And Acceptance Of Construction Works, Hereinafter Referred To As Stwiorb, Constituting Annex No. 8 To The Swz, And In Accordance With The Bill Of Quantities Constituting Annex No. 9 To The Swz. In Addition, The Ordering Party Stipulates That After Completing The Works Covered By The Subject Of The Order, The Contractor Is Obliged To Perform:9) Measurements, Tests And Checks Required By Regulations, As Well As An As-Built Inventory In The Scope Of All Works Performed, Including Electrical And Low-Voltage Installations.3. Due To The Fact That The Works Are Carried Out In An Active Facility, The Ordering Party Stipulates That The Contractor, Before Commencing The Works Covered By The Order, Is Obliged To Develop And Provide The Ordering Party With A Work Implementation Schedule. The Ordering Party Stipulates That The Contractor Is Obliged To Agree The Work Implementation Schedule With The Head Of The Technical And Operational Department Of The District Hospital In Zakopane Before The Commencement Of The Works, So As Not To Cause, Among Others, Excessive Inconvenience To Patients And The Work Of The District Hospital In Zakopane.4. A Detailed Description Of The Subject Of This Order Includes: 1) Drawing Documentation - Annex No. 7 To The Swz, 2) Stwiorb - Annex No. 8 To The Swz - In The Part Containing Sets Of Requirements For The Execution And Assessment Of The Correctness Of Execution Of Individual Works; 3) Bill Of Quantities - Annex No. 9 To The Swz.5.
