1. The Subject Of The Order Are Care Services For The Elderly And Disabled (Hereinafter Referred To As Services).2. Care Services Will Be Provided On The Basis Of The Act Of July 19, 2019 On The Provision Of Social Services By The Social Services Center For Persons Using Social Services (Hereinafter: Persons Or In The Singular Persons). 3. The Services Will Be Provided To The Social Services Center In Mikołów, Implemented In The Mikołów Commune In The Place Of Stay Of The Persons Covered By The Services, Taking Into Account The Close Surroundings As Part Of The Project Cus - A New Look At Social Services.4. The Forecast Number Of Care Services Is 7,500 Hours. Due To The Specificity Of The Services, The Ordering Party Informs That The Number Of Hours Given Above Is Forecast, I.E. It Is An Estimate And During The Period Of Order Execution, Their Number Will Depend On The Actual Number Of People And Their Needs, And Will Change During The Execution. The Ordering Party Will Pay Remuneration Only For The Actual Time Of Providing The Services. The Ordering Party Reserves The Right To Reduce Or Increase The Number Of Hours Of Services In Relation To Those Indicated Above, And The Contractor Is Not Entitled To Any Financial Or Legal Claims In This Respect. The Minimum, Guaranteed Scope Of Implementation Is Indicated In The Contract.5. The Scope Of Care Services Includes Assistance In Meeting Everyday Life Needs, In Particular: - Activities Related To The Provision Of Food Products - Purchasing Food, Keeping An Expense Report And Settling Accounts With The Person - Preparing Or Delivering Meals - Assistance In Eating Meals Or Feeding - Maintaining Cleanliness Of Tableware, Kitchen Utensils And Other Household Equipment Used By The Person - Keeping Sanitary Equipment And Sanitary Facilities Clean - Burning The Stove, Bringing Coal And Water - Assistance In Moving Around - Maintaining Cleanliness In Rooms Used By The Person (Current Cleaning) - Activities Related To Maintaining Cleanliness Or Changing Clothes, Personal Underwear, Bed Linen, Table Linen, Towels, Curtains, Curtains, Hanging Out Laundry, Sending The Bed - Ironing The Persons Basic Items - Washing Windows Once Every Six Months, Along With Removing And Installing Curtains And Drapes - Activities Related To Managing Personal Affairs Also Resulting From The Health Condition, Including Assistance In Dealing With Official Matters, Assistance In Accessing Health Services, Payment Accounts - Facilitating Access To Education And Culture, Maintaining Individual Interests6. 1 Hour Of Services Provided By The Ordering Party Means 60 Minutes Of Services Provided At The Persons Place Of Residence. The Provision Of Services Does Not Include The Time Needed To Reach The Place Of Service Or Travel Costs. Services May Also Be Provided On A Quarter-Hourly Basis. The Ordering Party Understands A Quarter Of An Hour As 15 Minutes. The Ordering Party Assumes That The Value Of 15 Minutes Of Providing Services Is ¼ Of
