The Subject Of The Order Is The Successive Delivery Of 77 Tons Of Chlorine In The Ordering Partys Barrels With A Capacity Of 500 Kg (Hereinafter Also Referred To As The Product) - Used To Disinfect Water Intended For Human Consumption By The Contractors Transport. 2. Place Of Delivery: Goczalkowice Water Treatment Plant, Ul. Jeziorna 5, 43-230 Goczalkowice-Zdrój (Pl22c) 3. A Detailed Description Of The Subject Of The Order Is Included In Annex No. 5 To The Swz. 4. Detailed Conditions For The Execution Of The Order Are Included In Annex No. 6 To The Swz. 5. The Means Of Proof In Question: 5.1. In Order To Demonstrate That The Offered Product Complies With The Requirements Specified In Annex No. 5 To The Swz - Description Of The Subject Of The Order, The Ordering Party Requests That The Contractor Submit The Following Evidence Together With The Offer: 1) A Current Hygienic Certificate Issued By The National Institute Of Public Health - National Institute Of Hygiene, Authorizing The Product For Use In Water Intended For Human Consumption; 2) Technical Specification Of The Offered Product. Note: The Technical Specification Should Confirm Compliance With All Requirements Specified In The Product Characteristics Included In Point 2 Of Annex 5 To The Swz. – Description Of The Subject Of The Order. The Ordering Party Requires That The Technical Specification Include Specific Numerical Values ​​For Each Of The Parameters Indicated In Point 2 Of Annex No. 5 To The Swz (Except For The Form). The Ordering Party Will Accept Equivalent Means Of Evidence If They Confirm That The Offered Deliveries Meet The Requirements Specified By The Ordering Party. 5.2 The Ordering Party Reserves That If The Contractor Fails To Submit The Evidence Referred To Above, Or If The Evidence Submitted Turns Out To Be Incomplete, The Contractor Will Be Summoned In Accordance With Art. 107 Section 2 Of The Act To Submit Or Supplement Them Within The Prescribed Deadline.
