Examination Tables – Delivery, Assembly, Installation And Commissioning Of Chemotherapy Chairs. 1. The Subject Of The Order Is: Delivery, Assembly, Installation And Commissioning Of Chemotherapy Chairs, Hereinafter Referred To As Equipment. 2. The Description Of The Subject Of The Order Is Included In The Required Parameters Form (Appendix 2 To Swz, Hereinafter Also Referred To As Fpw), This Form Must Be Submitted Together With The Offer Form (Appendix 1 To Swz, Hereinafter Also Referred To As Fo). 3. The Ordering Party Requires That The Offered Equipment Constitutes A Medical Device - Within The Meaning Of The Act Of April 7, 2022 On Medical Devices (Consolidated Text: Journal Of Laws Of 2024, Item 1620, As Amended, Hereinafter Referred To As: Uowm), Placed On The Market And Used In The Territory Of The Republic Of Poland In Accordance With Applicable Regulations. 4. The Ordering Party Requires That The Contractor, When Implementing The Subject Of The Contract, Act In Accordance With The Do No Serious Harm (Dnsh) Principle, Specified In Art. 17 Of Regulation (Eu) 2020/852, In Particular: • Ensured That The Devices Meet Energy Efficiency Requirements, • Ensured The Possession Of The Required Environmental Certificates, • Ensured The Minimization Of Emissions And Environmental Impacts, • Collected And Managed Waste Generated During Delivery, Installation Or Servicing In Accordance With The Waste Management Hierarchy (Prevention, Reuse, Recycling, Recovery, Disposal), • Transport And Installation Of The Devices Were Carried Out In A Manner Minimizing The Impact On The Environment. 5. The Ordering Party Pursuant To Art. 30 Section 2 Of The Public Procurement Act, Awards The Contract In Parts, Each Of Which Is The Subject Of A Separate Procedure. 6. The Ordering Party Does Not Allow The Submission Of Partial Offers In A Given Procedure. The Order Consists Of 1 Task. The Procedure Concerns The Purchase Of One Type Of Equipment. 7. The Ordering Party Does Not Allow The Submission Of Variant Offers.
