The Subject Scope Of The Order Includes The Following Medical Examinations And Tests: Examination By A Specialist In Internal Medicine, Including: General Examination, Measurement Of Blood Pressure And Electrocardiogram (Ecg), With Frequency Once During The Term Of The Contract; In Connection With Work With Video Displays And Visual Strain - Examination By An Ophthalmologist, With Periodicity Annually. Examination Of Visual Acuity And Refraction To Register Diseases Of The Eye And Its Appendages And Deviations In Visual Acuity. If Necessary - Issuing A Prescription For Glasses; Clinical Laboratory Tests - Complete Blood Count, Blood Sugar, Cholesterol, Triglycerides With Frequency Once During The Term Of The Contract; The Deadline For The Execution Of The Public Procurement Is 24 (Twenty-Four) Months From The Conclusion Of The Contract. Periodic Medical Examinations And Tests Will Be Carried Out According To A Schedule Agreed Upon At The Conclusion Of The Contract Between The Contracting Authority And The Contractor. The Schedule For Carrying Out The Medical Examinations And Tests Will Be Prepared By The Contractor Together With The Territorial Divisions Of The Plovdiv Region Within 14 Days Of Signing The Contract For Awarding The Execution Of The Public Procurement. The Performance Of The Services Must Begin Within 30 (Thirty) Days Of Signing The Schedule. The Estimated Number Of Workers And Employees Included For Service In The Public Order Is 471 (Four Hundred And Seventy One) Numbers. The Participant Selected As A Contractor Must Submit A Preventive Card For The Medical Examinations Performed For Each Worker And Employee With A Conclusion On The Health Status Of The Worker And Employee. The Transfer Of The Performance Of The Services Is Documented By Drawing Up A Handover Protocol, Which Contains A Description Of The Services Actually Performed, Accompanied By A List Of The Persons Who Have Passed Medical Examinations And Tests, Which Is Signed By Representatives Of The Contractor And The Contractor In Two Original Copies - One For Each Of The Parties.
