1. The Subject Of The Order Is The Provision Of Services Consisting In Providing A Temporary Place In A Shelter With Care Services For Homeless People Referred By The Municipal Social Welfare Center In Kalisz, In Accordance With The Provisions Of Art. 48A Of The Act On Social Assistance Of March 12, 2004 (Journal Of Laws Of 2025, Item 1214, As Amended), Along With Ensuring The Necessary Social Conditions And Care Services For Homeless People, Including Full Board And Social Work For Homeless People Who, Due To Age, Long-Term Illness Or Disability, Require Partial Assistance And Care From Other People (Including Care Nursing) Aimed At Helping In Dealing With Basic Life Matters. Providing Activation Services Aimed At Developing Or Strengthening Social Activity, Gaining Independence In Life And Ending Homelessness. Providing Information About Available Forms Of Assistance And, If Necessary, Helping To Organize It.2. The Ordering Party Anticipates That The Estimated Estimated Number Of People Sent To The Shelter During The Contract Period Will Be 11 People (It Is Assumed That It Will Not Be Less Than 10 People). 3. The Estimated Volume Of The Order In The Period From January 1, 2026 To December 31, 2026 Is An Estimated 3,654 Services.4. The Standard Of Basic Services Provided In A Shelter For Homeless People, Qualifications Of People Providing Services There And The Standard Of The Facility In Which The Shelter For Homeless People Is Located Must Also Be Consistent With Annex No. 2 To The Regulation Of The Minister Of Family, Labor And Social Policy Of April 27, 2018 On Minimum Standards For Night Shelters, Shelters For Homeless People, Shelters For Homeless People With Care Services And Heating Rooms (Journal Of Laws Of 2018, Item 896, As Amended).5. Due To The Specificity Of The Order, Which Is Characterized By Variability Of Customer Needs Due To Their Changing Health Condition And Personal Situation, The Number Of Homeless People Provided By The Ordering Party Is The Expected Number For The Entire Duration Of This Order. The Ordering Party Reserves The Right To Reduce Or Increase The Above. Number Of People Depending On The Actual Needs In This Regard. The Contractor Declares That He Accepts The Above Reservations And Will Not Be Entitled To Any Claims, Including Monetary Ones, Against The Ordering Party.6. The Ordering Party Will Pay For The Actual Number Of People Using The Services Provided By The Shelter. 7. Each Time A Person In Need Of Shelter Will Be Placed On The Basis Of A Referral To A Shelter And An Individual Administrative Decision To Grant Assistance In The Form Of Shelter, Issued By The Director Of The Municipal Social Welfare Center In Kalisz Or Another Authorized Person. The Decision Will Include: Name And Surname Of The Beneficiary, Type, Scope And Period Of Service Provision. 8. In Particularly Justified Cases, The Contractor Will Provide Services Based On Data Provided By Telephone, Fax Or E-Mail. Such Service Request Will Be
