1. The Subject Of The Order Is The Provision Of Services For Manual Dismantling Of Bulky Waste And Sorting/Finishing Of Construction Waste At Zakład Gospodarki Odpadami S.A. In Bielsko-Biała At Ul. Krakowska 315 D.2. The Scope Of Services Includes: 1) Manual Dismantling Of Large-Size Waste Using Simple Tools Provided By The Contractor (Hammers, Axes, Crowbars, Hand Saws, Etc.) And Loading Of Separated Raw Materials Into Appropriate Containers; 2) Manual Sorting/Finishing Of Construction Waste; 3) Maintaining Order At The Workplace And The Adjacent Surroundings; 4) If It Is Not Possible To Perform Bulky Waste Dismantling And Manual Sorting/Cleaning Services Construction Waste, The Ordering Party May Commission The Contractor To Work At The Composting Plant, Operating The Glass Or Styrofoam Sorting Line, Including Operating The Briquetting Machine. 3. The Service Will Be Provided: 1) By 3 Trained Employees; 2) Monday To Friday From 7 A.M. To 3 P.M. The Ordering Party Reserves The Right To Modify The Working Time (Time Of Providing Services), Including The Number Of Hours Per Shift, Of Which It Will Inform The Contractor At Least 24 Hours Before The Planned Modification.4. The Estimated Number Of Man-Hours Per Employee In Providing The Service Being The Subject Of The Contract Is 160 Man-Hours Per Month (1,920 Man-Hours Over A 12-Month Period). The Ordering Party Stipulates That The Given Number Of Man-Hours Is An Estimate And May Be Changed Depending On The Ordering Partys Needs Without Financial Consequences. The Ordering Party Ensures That The Minimum Number Of Man-Hours For All Employees Will Be: 2,000.5. Pursuant To The Provisions Of Art. 95 Of The Public Procurement Law, The Ordering Party Requires That At Least 3 Persons Participating In The Execution Of The Order, Performing Activities Necessary For The Implementation Of The Contract, And Regarding Activities Related To The Manual Dismantling Of Large-Sized Waste And The Sorting/Refining Of Construction Waste, Be Employed Under An Employment Contract Within The Meaning Of The Act Of June 26, 1974 - Labor Code (Journal Of Laws Of 2025, Item 277, As Amended). D.). The Employment Obligation Applies Throughout The Entire Period Of Performance Of The Above-Mentioned. Activities During The Term Of The Contract And Applies To Both The Contractor And The Subcontractor. 6. The Contractor Provides Protective Vaccinations For All Employees (Regardless Of The Basis Of Employment), In Particular Against: Jaundice, Tetanus, Typhoid Fever, Hepatitis A, Hepatitis B.7. The Contractor Takes Full Responsibility For Matters Related To Occupational Health And Safety Of Employees Involved In The Implementation Of The Contract.8. The Contractor Is Obliged To Comply With All Applicable Legal Provisions And Special Conditions And/Or Internal Regulations In Force On The Premises Of The Ordering Party, And Also Ensure That Their Employees Comply With Them.
