The Subject Of The Order Is The Supply Of 44.4 Tons Of Activated Powdered Carbon - Used For The Treatment Of Drinking Water (Hereinafter Referred To As The Product). 2. The Scope Of This Task Will Be Completed In Its Entirety. 3. A Detailed Description Of The Subject Of The Order (Opz) Is Included In Annex No. 5 To The Swz. 4. Detailed Terms And Conditions Of The Order Are Set Out In Annex No. 6 To The Swz. 5. The Means Of Proof In Question: 5.1. In Order To Demonstrate That The Offered Product Complies With The Requirements Specified In Annex No. 5 To The Swz - Description Of The Subject Of The Order, The Ordering Party Requests That The Contractor Submit The Following Evidence Together With The Offer: 1) A Current Hygienic Certificate Issued By The National Institute Of Public Health - National Institute Of Hygiene, Authorizing The Product For Use In Water Intended For Human Consumption; 2) Technical Specification Of The Offered Product. Note: The Technical Specification Should Confirm Compliance With All Requirements Specified In Point 2 Section 2.1. Annex No. 5 To The Swz - I.E. In The Description Of The Subject Of The Contract. The Ordering Party Requires That The Technical Specification Include Specific Numerical Values ​​For Each Of The Parameters Indicated In Point 2. 2.1. Annex No. 5 To The Swz [Except Data Regarding Origin]. The Ordering Party Will Accept Equivalent Means Of Evidence If They Confirm That The Offered Deliveries Meet The Requirements Specified By The Ordering Party. The Ordering Party Allows Parameters To Be Provided In Units Other Than Those Indicated In The Product Characteristics, Provided That They Can Be Converted Into The Units Specified Therein. The Evidence In Question And Other Documents Or Statements Prepared In A Foreign Language Are Submitted Together With A Translation Into Polish. 5.2. The Ordering Party Stipulates That If The Contractor Fails To Submit The Evidence Referred To Above, Or If The Evidence Submitted Turns Out To Be Incomplete, The Contractor Will Be Summoned In Accordance With Art. 107 Section 2 Of The Act To Submit Or Supplement Them Within The Prescribed Deadline.
