The Subject Of The Order Is The Transport Service Of Central Sterilization Containers And Medical Products. 2. Name Of The Item According To The Common Procurement Vocabulary: Cpv 60130000-8 - Transport Services. 3. A Detailed Description Of The Subject Of The Order, Estimated Quantities And The Conditions Under Which The Subject Of The Order Will Be Implemented Are Set Out In The Annexes (Description Of The Subject Of The Order, Draft Contract, Offer Form). 4. As Part Of The Service Provided, The Contractor Is Obliged To Apply And Comply With The Legal Provisions Applicable To The Subject Of The Order, Have Appropriate Authorizations And Bear Full Responsibility In The Event Of Their Violation. 5. Pursuant To Art. 95 Section 1 Of The Public Procurement Act, The Ordering Party Requires The Contractor Or Subcontractor To Employ Under An Employment Contract Within The Meaning Of The Act Of June 26, 1974, Labor Code, Labor Code (Journal Of Laws Of 2019, Items 1040, 1043 And 1495) Continuously Throughout The Duration Of The Agreement, Persons/Persons Performing Activities Related To The Implementation Of The Subject Of The Order. 6. No Later Than The Conclusion Of The Contract, The Contractor Will Provide The Ordering Party With A Declaration Of Compliance With The Requirement Referred To In Section 5, I.E. That The Person/Persons Performing Activities Related To The Implementation Of The Subject Of The Order Will Be Employed Under An Employment Contract Within The Meaning Of The Act Of June 26, 1974, Labor Code. 7. At Each Stage Of Contract Implementation, The Ordering Party Has The Right To Verify The Contractors/Subcontractors Declaration (Concerning The Form Of Employment Of The Persons Referred To In Section 5). For This Purpose, He Is Entitled In Particular To Request The Competent Labor Inspectorate To Conduct An Inspection. 8. The Ordering Party Does Not Reserve The Right To Apply For The Order Only By Contractors Referred To In Art. 94 Of The Public Procurement Law. 9. The Ordering Party Does Not Impose The Requirements Referred To In Art. 96 Of The Public Procurement Law. 10. The Contractor Is Obliged To Have Voluntary Liability Insurance With The Value Indicated In The Draft Contract. No Later Than Before Concluding The Contract, The Contractor Will Provide The Ordering Party With Documents Confirming The Fulfillment Of This Requirement.
