A Detailed Description And Method Of Implementing The Subject Of The Order Is Set Out In Annex No. 1 To Swz.2. The Implementation Of The Subject Of The Order Is Covered By Co-Financing From The European Social Fund Plus Under The European Funds For Małopolska 2021-2027 Program, Priority 6 European Funds For The Labor Market, Education And Social Exclusion, Measure 6.10 Support For General Education, Project Type A. Inclusive Education In Schools And Educational System Institutions Providing General Education.3. The Ordering Party Requires The Contractor To Submit A Substantive Report On The Implementation Of The Order According To The Template Constituting Annex No. 9 To The Swz Together With Attachments In The Form Of: A) A List Of Names Of Persons Covered By Support, Indicating The Type Of Support, The Date Of Its Implementation And The Number Of Hours Of Support Provided B) A Substantive Description Of The Support Provided; The Ordering Party May Also Request From The Contractor Other Information Necessary To Submit A Payment Application Required By The Intermediate Body In The Process Verification And Approval Of The Payment Application.
